Click here to complete the Communications Request Form (CRF). Read the FAQ below before filling it out.
Here are some frequently asked questions:
Who should fill out a CRF?
Staff members and ministry leaders. It might make sense for a volunteer to complete the form; this should only be done after that volunteer has met with a staff member or ministry leader. The staff member or ministry leader should serve as the conduit between volunteers and the Communications Team.
When should I complete a CRF?
We need CRFs at least TWO weeks prior to when you want communication started.
What about a video project?
We require notification of video production at least SIX weeks prior to when you want a video. Send that request to Claire.
Are there any exceptions to filling out a CRF?
Yes: sympathy notices, baby shower announcements, or birth & wedding announcements should be sent directly to the Pastors.
If you need something edited on the website send that directly to Claire.
If you’re hosting a bigger event (conference, Missions Weekend) first set up a meeting with the Communications Team to talk through your needs. Email the Pastors or Claire.
Can the CRF be edited after it is submitted?
No. All changes (date, time, contact info) should be sent directly to communications@faithliveitout.org. Don’t submit a CRF until you have all of the details. (If we don’t have all the details, there’s not much we can do with it.)
How do I save or print my CRF as a PDF?
BEFORE you submit your form, right click on the form and either save as or print the form.
